Individual and Group Consultations

Certified Advance Care Planner Jai Miranda will meet with you to introduce the process of Advance Care Planning, help you through each step of the way, and ensure you have what you need to be sure documents are on file with your medical team and others who play an important role in your care. Jai can also engage your organization or congregation in conversations about how you can promote end of life discussions among you members.

Jai is a Conversation Project Champion, using resources and tools from the Conversation Project, a public engagement initiative of the Institute for Healthcare Improvement (IHI). Dusk to Dawn embraces IHI's simple and transformative goal: to help everyone talk about their wishes for care through the end of life, so those wishes can be understood and respected.
